Your monthly Explanation of Benefits (EOB) shows a summary of the claims we received the previous month, what we paid and what you owe. You’ll get a new one each month you have claims.
Your EOB shows a summary of the claims we received, what we paid and what you owe. EOB statements posted to this site will look slightly different than EOB statements that included a check. The electronic copies do not include an image of the check.

Your Prescription Drug EOB statement can help you keep track of your monthly prescription drug costs. It has helpful definitions, drug list updates, phone numbers to call for help and other useful information. Below are descriptions of each section of the Prescription Drug EOB.
Section 1: Your prescription drugs during the past month. This section lists the covered Part D prescription drugs you filled in the past month. It also gives a breakdown of drug payment amounts (what the plan paid, what you paid and any other payments made for the drug). Please look over this information to be sure it’s correct.
Section 2: Your drug payment stage. This section shows you which drug payment stage you’re in. The amount you pay for your prescriptions depends on which drug payment stage you’re in when you fill your prescription. You move from one stage to the next based on how much is spent for your drugs.
Section 3: Your out-of-pocket costs and total drug costs. This section defines and tracks your out-of-pocket costs and total drug costs. These costs determine which drug payment stage you’re in.
Section 4: Updates to the plan’s Drug List for drugs you take. This section has changes to the coverage or cost of your plan-covered drugs
Section 5: What to do if you see mistakes or have any questions. If something doesn’t look right, or you don’t understand how your drug plan coverage works, contact us
Section 6: Important things to know about your drug coverage and your rights. Your Evidence of Coverage has details about your drug coverage and costs. It also explains the rules you need to follow when using your drug coverage.
